Self Eligibility Screening

We highly recommend that you complete this self screening assessment to determine whether the supportive housing that you provide generally appears to be a "recovery residence" or "recovery housing" before investing significant time in completing the application. A warning will appear at the bottom of this page, if you answer one or more of the following questions in a way that would indicate that you do not operate a recovery home or recovery residence. Regardless of whether you get a warning or not, there is no guarantee that your application will result in certification.

Do you currently provide a shared living environment centered on peer support and connection to services that promote sustained recovery from substance use disorders?

Must all eligible residents have an addictive disorder?

Abstinence Test(Required)

Must all residents abstain from alcohol and illicit substances?

Yes.

No.

Accountability Test(Required)

Do you use drug testing protocols to ensure a safe, sober living environment?

Yes.

No.

Based on one or more of your answers, your housing does not appear to be a recovery residence or eligible for certification. If you submit an application with payment, you are not guaranteed to become certified. Refunds are not provided.
